What Is Hypertension Staging?

Definition

Chinese guideline staging: normal <120/80; high-normal 120-139/80-89; grade 1: 140-159/90-99; grade 2: 160-179/100-109; grade 3: ≥180/110. Home readings subtract 5 mmHg for white-coat effect (≥135/85). Staging is only the start: with diabetes, kidney disease or prior MI, even grade 1 is managed as high risk. Targets are generally <140/90, or <130/80 if tolerated. Lifestyle measures (salt <5 g/day, weight loss, exercise, alcohol limits) each lower BP by 2-8 mmHg.

Formula

Stage by the higher of systolic or diastolic category

Worked Example

A one-week home average of 146/92 (grade 1): start with 3 months of lifestyle change (5g less salt + 150 min/week brisk walking + 5% weight loss); medicate only if still ≥140/90.

Common Misconception

"No symptoms, no harm" — 70% of hypertension is silent while target organs (heart, brain, kidney, eye) accrue damage. "Young people don't get it" — cases under 40 have doubled in a decade on sedentary high-sodium late-night lifestyles. And stopping meds once readings normalize causes rebound spikes — a common trigger of hemorrhagic stroke.
